Rates are age-standardized to the 2000 projected U.S. Census Bureau population aged ≥45 years
using three age groups: 45–64, 65–74, and ≥75 years. Persons of Hispanic or Latino (Hispanic) origin might be of any race but are categorized as Hispanic; all racial groups are non-Hispanic.
