In 2022, 64 percent of chronic condition special needs plans (C-SNPs) provided coverage to beneficiaries with diabetes, while coverage for end stage renal diseases was only provided by four percent of C-SNPs. C-SNPs are Medicare Advantage plans tailored to provide coverage to beneficiaries with chronic disease conditions in the United States.
